Our role is to understand what matters most to you, and to ensure that care is delivered in a way that reflects your wishes, preferences, and priorities.

Working alongside families and professionals 

Palliative care is never provided in isolation.

We work closely with families, as well as healthcare professionals such as district nurses and other medical teams, to ensure care is coordinated and responsive.  

Families are often navigating a very emotional time. Our role is to support not only the individual, but also those around them, offering reassurance, guidance, and practical help.

This allows families to focus on being present with their loved one, rather than managing care alone.

Responsive care when it is needed most

Situations requiring palliative care can change quickly.

We understand the importance of responding without delay, particularly when care is needed urgently or following a change in condition.

We can typically begin care within 72 hours, and in urgent situations, within 24 hours where appropriate.

Our focus is on ensuring that support is in place quickly, so that comfort and reassurance are never delayed.

Care shaped around your wishes

Palliative care is deeply personal.

We take time to listen carefully, understand your preferences, and build a care plan that reflects how you wish to be supported. This includes how care is delivered, who is involved, and what matters most during this time.

 Care plans are reviewed regularly and adjusted as needed, ensuring support remains aligned with your wishes as circumstances change.

A calm and supportive process from the start

Arranging palliative care can feel overwhelming, but it does not have to be.

We guide you through each step, beginning with a conversation to understand your situation. A detailed assessment follows, allowing us to build a personalised plan of care.

 Once agreed, your care team is introduced and support begins.

 We remain available throughout, providing ongoing support, reassurance, and adjustments whenever needed.
