Home care referrals and partnerships

A reliable care partner for referrals, discharge and ongoing support

Working with healthcare professionals to deliver responsive, person-centred home care that supports safe discharge and ongoing care.

Supporting

professionals with safe and reliable care

Arranging care for individuals can often come with time pressure, complexity, and the need for absolute reliability.

Whether supporting hospital discharge, coordinating ongoing care, or responding to urgent referrals, it is essential to work with a provider who can act quickly and deliver consistently.

At Integral Care, we work closely with healthcare professionals, social workers, and commissioning teams to ensure care is delivered safely, efficiently, and in line with individual needs.

Experienced in working with local authorities and the NHS

We are commissioned by local authorities and work in partnership with NHS Continuing Healthcare teams across Northamptonshire.

This includes working with:

  • Social workers and care coordinators
  • Brokerage teams
  • Hospital discharge teams
  • Continuing Healthcare (CHC) services

This experience allows us to understand processes, expectations, and the importance of clear communication when managing referrals.

Responsive

support when timing matters

Many referrals require a timely response.

Hospital discharge delays, changes in condition, or urgent care needs often mean that care must be arranged quickly and with confidence.

We can typically begin care within 72 hours, and in urgent situations, within 24 hours where appropriate.

From initial referral through to care delivery, we act quickly to assess needs and allocate appropriate support.

Delivered by carers you can trust

Personal care is one of the most personal forms of support someone can receive.

That is why we place such importance on who provides your care. We recruit carers for their compassion, empathy, and ability to connect with people on a human level.

Every member of the team is fully trained, carefully selected, and introduced properly, so you feel comfortable and reassured from the very beginning.

Where possible, we provide a consistent team of carers, allowing you to build trust and familiarity over time.

Care that is person-centred and outcome-focused

While we work within structured frameworks, our approach remains focused on the individual.

We take time to understand not only clinical needs, but also personal preferences, routines, and what matters most to each person.

This ensures care supports both outcomes and quality of life, helping individuals remain safe, comfortable, and as independent as possible.

Ongoing communication and review

Clear communication is essential when working with professionals.

We maintain regular contact through:

  • 72-hour initial reviews
  • Ongoing care plan reviews every 4 to 6 weeks
  • Weekly internal care discussions and oversight
  • Responsive updates where changes occur

This ensures that care remains aligned with evolving needs and that any concerns are addressed promptly.

Kind words

We have a track record of providing exceptional care.

I cannot speak highly enough of the carers of this company. All carers - of both sexes - were caring and professional. Nothing was too much trouble for them.

They attended four times a day throughout the coronavirus problems, and when we went out to clap on a Thursday, I was clapping for them. My wife would have also agreed with the above.
